You will need to complete and upload the following forms as part of your liquor license application. The forms can be found on our webpage and are also linked below. Each form will need to be saved and uploaded into this online form using the upload buttons below. After all forms are uploaded, you will be taken to the payment screen.
1. Liquor license application (new or renewal)
2. Auxiliary questionnaire (required for each officer listed on the application)
3. Appointment of Agent (also needs Auxiliary questionnaire form)

For new applications, please contact the city clerk for the fee amount to enter onto the payment page. You may also mail a check to our office at 7426 Hubbard Avenue, Middleton, WI 53562.
