The Middleton Chamber of Commerce is partnering with the City of Middleton to gather private business damage assessment forms for the purposes of an emergency management declaration to the state and federal governments. If you represent a business in Middleton that has experienced damage due to the flooding, please provide your information in the form linked below and return it to chamber@middletonchamber.com as soon as possible. We anticipate needing this information by noon Wednesday.
