Dog Licenses / Park Permits

State and municipal law (City of Middleton Code - Chapter 7) requires that all dogs five months and older be licensed with the city each calendar year. If you are licensing your dog for the first time or you have recently vaccinated your dog, a rabies certificate must be presented at the time of licensing. The city is proud to offer low-cost fees for licensing as listed below.

Licensing your dog is a requirement of the city. If your pet were to stray, the tag is a valuable resource to help bring them back home quickly. Cats do not need to be licensed.

Dog & Cat Limit: 16.05(17) No person shall harbor, shelter, or possess more than five dogs or cats or any combination thereof upon a single premise,

Dane County Dog Park Permits are also available. They are good at all Dane County Dog Parks for the calendar year. You may apply for a dog park permit online when applying for your dog's annual license.

  • All Dog Licenses: $25
  • Annual Dog Park Permit: $43
  • Senior / Disabled Dog Park Permit (65+): $20 (must apply/pay in person to receive this discount)
  • Additional Dog Park Permit or Replacement Permit: $20

Apply / Renew Online

When applying online you will need to:
  • Complete and submit the Dog License Application Form
  • Upload a rabies certificate for each dog
  • Make Payment Online via credit card or EFT (fees apply) If you would like to pay by a check, please put a check in the City drop-box at 7425 Hubbard Avenue and label the envelope "Dog License"
  • *NOTE* your dog(s) are not licensed until we receive payment

Dog Parks

Please see the Middleton Parks & Recreation Department for more information on the City of Middleton Dog Parks.
Daily Dog permits are $8 and must be paid for using the electronic pay stations located at the Dog Parks.  For annual park permits save your receipt to use as a temporary permit until you are able to redeem.
