Register By Mail Or In-Person


To register by mail or drop-off, please complete the registration form. We do not accept phone registrations.

Late Registration


Late registration and registration at the program will not be accepted; no exceptions. Registration deadlines are important, so please register by the posted deadlines so staff can properly prepare for the program.

Scholarships


If you are interested in applying for a scholarship, you can contact our office at 608-821-8360 for more details, or print off the application.

Transfers


Transfers between classes must be done within a minimum of one week prior to the start of class. After that time, transfers will not be accepted.

Refunds


Refunds will be made on all classes that are cancelled by the Recreation Division. Full refunds will be honored for any written request made a minimum of one week prior to the start of the class or program. Credit card processing fees are non-refundable. Refunds will not be issued for unattended classes, weather related cancellations, or holiday dates. A $5.00 service charge will be withheld for all refunds, unless the activity is cancelled by the Recreation Division. There will be no refunds issued once the program starts. Participants who miss classes due to illness, vacations, etc. will not be provided the opportunity to make them up.

Concussion Law


Due to the increase in concern for concussions in youth sports, there is now a Wisconsin Concussion Law. Our department requires all participants to review the
Concussion Agreement and Information prior to registering for programs. Our participant safety is our number one concern!

Credit Cards


We accept Visa and Mastercard online and in-house. All in-house and online credit card payments are charged a non-refundable processing fee. This is charged by the processing company, not Middleton Recreation.

Program Postponements/Inclement Weather


Call our HOTLINE at 608-821-8357, visit the Public Lands, Recreation, and Forestry page, or Facebook to see the status of a program due to inclement weather. Any programs postponed due to inclement weather will not be rescheduled. If MCPASD closes, no programs will be held at the schools.

Min/Max Enrollment


Middleton Recreation will determine the minimum number of participants needed to hold the program, and reserves the right to cap enrollment when deemed necessary. If the required minimum is not met, the program will be cancelled.

Confirmation of Enrollment


Once you have registered, plan on attending at the listed times and dates. Please feel free to call to verify enrollment and/or ask questions. The Recreation Division will only contact you if the program is full, cancelled, or there are changes.

Age/Grade Requirements


A child must meet the required program age by the first day of the program. All grade restrictions will be based on the 2020-2021 school year. Age and grade requirements are set to benefit all children and to make instruction easier for the program leader. All participants must register for the correct class or they may be contacted to move classes or lose their spot in the program.

Resident/Non-Resident


Residents are those that reside within the City of Middleton or within the MCPASD boundaries.


Those who do not live within the 53562 zip code, but reside within the school district boundaries, must contact the Recreation Office prior to registering to manually set-up your account and adjust your residency status.  If you do not contact the office prior to registering, the system will charge you as a non-resident and non-resident fees will not be refunded.

Photo Policy


Middleton Recreation Division programs and events are often photographed for promotional purposes. If you do not wish to be included in a Recreation Division photo, please indicate this to the instructor and/or photographer.

Liability


Participants must assume full responsibility for injuries while participating.
